Job Introduction

I am a gentleman in my 40’s living in my own home, and I have a diagnosis of Autism Spectrum Condition. I have spent a lot of my life in hospitals but I have now moved into my own home in Whitegate, Cheshire, with the support of my team from Gray Healthcare.  I’m currently looking for an experienced Team Leader to join my team. I have two staff with me during the day and night, to support me to keep safe and well.

Job Introduction

I’m a very sociable man and I enjoy speaking to people, so it would be great to have staff who are also chatty and outgoing. I have a very good sense of humour and I enjoy keeping to my routine.  I need staff who can positively motivate me to keep me engaged with my routines and stay active.

I need staff who have experience with positive behavioural support plans who can help identify my triggers and are able to offer me reassurance.  There are many things that cause me to feel anxious or distressed but my staff team help me to manage these situations.

I have a diagnosis of Dysphagia which affects my eating, drinking and swallowing significantly. I am at risk of choking and/or aspirating food or fluid onto my lungs. I can sometimes regurgitate my food and fluids and this requires robust care and attention from staff working with me.

I have difficulty understanding others views and I need my team to keep their requests and instructions simple and be consistent in the way they word things.

Main Responsibilities

As a Team Leader within Gray Healthcare, you will be a front-line leader working alongside a specialist team to support an individual within their own home. We support people with complex needs, who may have a Learning Disability, Autism, Personality Disorder, Acquired Brain Injury (ABI), Substance Misuse issues, and people who require support with their Mental Health.

You will lead your team to deliver outstanding care, assuring quality and safety. You will develop and review support plans playing an integral role in the evaluation of these with the Multi-disciplinary team. You will be competent in IT systems; including electronic rostering and digital records. You will also spend time working shifts to develop trust and rapport with the person you support whilst leading by example.

The Ideal Candidate

The successful candidate will be providing direct management flexibly over a 24-hour period and providing quality assurance and full rota management ensuring safe, responsive and effective working practice. There is a requirement to work as part of his team and you may need to cover shifts at times to ensure that the gentleman receives the prescribed levels of support.

You will lead and role model for his team, oversee the rota, assist with the development of recovery plans, ensure that the PBS model is embedded within the service in line with the gentleman's strengths and needs and report on progress, risks, challenges and successes to both internal and external MDTs.

You will need to be a natural leader with great communication skills and possess an NVQ/QCF 3 in Health and Social Care or willing to work towards.

You must have a working knowledge of the Positive Behaviour Support Model (qualifications would be advantageous but not essential), be experienced in taking positive risks to enable an increased level of independence and be solution focussed when presented with challenging situations.
